- New Patient Parent Letter: Please read this carefully before your child's first appointment regarding our requirement for younger children to be supervised during the adult portion of the initial interview. The letter contains our requirements and what to do if you can not arrange supervision.
- Medical Records Request
- Stimulant Medication Request Certain stimulant medications require a hard-copy prescription (they can not be phoned into the pharmacy). If you are not due for a follow up appointment, you may mail in a refill request form with a self-addressed stamped envelope. Please see the form for more important details.
